Skilled Nursing Facilities are specifically for patients that require a medical need that is greater than what a senior living community can provide. If a client requires a Skilled Nursing Facility, Churchill SCM will help you find a facility that fits your need. Most Skilled Nursing Facilities accept Medicare/Medi-Cal.

Memory care communities are a wonderful option for those that have wandering or exit seeking behaviors and/or have a diagnosis of Alzheimer's or dementia. Memory Care is not for everyone so it is important to receive a proper assessment to determine if memory care should be an option. Memory care communities are typically private pay only and do not accept insurance as a form of payment.

Home Care is for those not ready to move from their home and can have an option of having a caregiver come to their home in shifts. These shifts can be every 4, 8 or 12 hours depending on the need. They can also vary to certain days of the week or
full time. Some Home Care Agencies accept insurances, others are completely private pay.
